A Little About Sandra

Sandra Hammel is a Registered Nurse and experienced case manager with a diverse background spanning clinical care, home health leadership, medical device support, and patient advocacy. She currently serves as the Owner and RN Case Manager of Springwell Case Management, where she works closely with individuals recovering from motor vehicle injuries to coordinate complex care plans, monitor medical progress, and guide patients through the healthcare system to support recovery and independence.

Sandra brings extensive clinical and operational experience to her role. Prior to founding Springwell Case Management, she served as Director of Clinical Care at Samaritas Home Health, overseeing clinical operations for a large home health program serving up to 100 community-based patients. 

Sandra’s clinical foundation includes years as a Hospice Case Manager with Reverence Home Health and Hospice, where she managed complex end-of-life care for terminally ill patients while supporting families through difficult medical decisions. She began her nursing career at St. John Hospital and Medical Center providing orthopedic and neurosurgical care.

Before entering the nursing profession, Sandra worked as an educator, teaching Spanish at the University Liggett School and serving as an elementary school teacher in New York City through the NYC Teaching Fellows program.

Sandra earned her Bachelor of Science in Nursing from Wayne State University, graduating with Dean’s List honors, and holds a Master’s degree in Urban Education from Long Island University. She also earned a Bachelor of Arts in Psychology with a minor in Spanish from Georgetown University.

With a unique combination of clinical expertise, leadership experience, and a deep commitment to patient-centered care, Sandra is dedicated to helping individuals navigate complex medical situations while advocating for the highest quality care and outcomes.
